A consultation is planned on a shake-up of day care services for elderly people and those with learning disabilities across Pembrokeshire.

The County Council is planning on “transforming day opportunities” with a new model of services focusing on four multi-use ‘wellness hubs’ for specialised care and support.

“Satellite provision” which will include links to “thriving community activities and opportunities in areas such as Milford Haven, Solva/St Davids, Fishguard and Crymych,” reducing the need for travel, a report to cabinet states.

The hubs are likely to be based at the Cleddau River Day Unit or The Anchorage social activity centre (SAC) in Pembroke Dock, The Avenue SAC in Tenby, Meadow Park Day Centre or Portfield SAC in Haverfordwst and Bro Preseli Day Centre, Crymych.

The hubs will aim to allow residents to “access care and support to meet people’s assessed needs in a more holistically, and more seamless way benefiting both physical and mental health and well-being.”

Cabinet member for social services Cllr Tessa Hodgson said it was an opportunity to develop a model “more bespoke for individuals.”

She added that the model had been developed in consultation with service users and their families and carers, with 234 people attending 13 co-production events.

Cabinet approved on May 13 to undertake a 12 week public consultation on the proposed new model for delivering day opportunities in Pembrokeshire.
